Bulletin of typical workplace accidents in Shanghai, July 2022
Overview
In July 2022, there were 27 fatal workplace accidents in Shanghai's industrial, mining, commercial and trading sectors with 27 deaths.
Typical Accidents in July
On July 5, at Floor 5 of No.3457 Caobao Road, an electrician of Shanghai Liancheng Dining Co. Ltd. was checking the wires in a dropped ceiling. The worker was suspected to have touched a live electrical wire and died.
On July 6, at the decoration site of Floor 1 of No.171 Yuanping Road, a worker was passing on construction materials on a scaffold tower. The worker had an electric shock and died.
On July 14, outside the No.2 dormitory building of Shanghai University Campus at No.20 Chengzhong Road, a worker of Shanghai Shenqiang Machinery Construction Co. Ltd. was installing a tie-in for a construction hoist. The worker fell off from the tie-in, which was about 14 meters above ground, and died.
On July 29, at No.50 Zhangwu Road, a worker from Shanghai Fuxin Cooling Technology Co. Ltd. was cleaning a central air conditioning system outlet in a room. The worker had an electric shock and died.
On July 31, at the R&D department of Shanghai Qifan Cable Co. Ltd. on No.238 Zhenkang Road, a worker lied unconsciously on the ground with an electrical fan fallen onto the body. The worker was suspected to have had an electric shock.
Typical workplace accidents in previous Julys
“2020.7.31” Fall from height accident at Fuquan Road and Jinzhong Road
Summary of the accident：
At about 1:40 am, July 31, 2020, at the IKEA shopping center construction site near the crossroad of Fuquan Road and Jinzhong Road, Changning District, Shanghai Qunli Industrial Co. Ltd. was pouring concrete to build the fourth level of underground support beam. A worker was standing on a higher-level beam to help relieve a stuck pump pipe. The worker lost balance, fell to the bottom of the pit and died.
Direct cause：
The worker was not wearing safety belts and was violating safety protocols by standing directly on the beam. There were no guardrails or protective equipment installed at the job site.
Indirect cause：
Shanghai Qunli Industrial Co. Ltd. has not implemented a system to hold people responsible for safety, did not effectively supervise the safety work of the pit construction team. The company did not have a proper safety briefing between shifts and did not identify and eliminate work safety hazards. The company did not properly manage job site safety issue, did not assign a dedicated safety supervisor, did not spot and stop the worker’s dangerous behavior.
Punishment for the accident：
District emergency management bureau imposed administrative penalties upon Shanghai Qunli Industrial Co. Ltd. and its project manager.
Recommendations：
Shanghai Qunli Industrial Co. Ltd. shall learn from this accident and amplify its safety review efforts to identify and eliminate work safety hazards. The company shall design and strictly implement a job site safety supervision program for projects with significant safety hazards. It shall strengthen its job site safety management capability and implement a system to hold people responsible for safety. It shall enhance its internal safety training programs to improve employees’ safety and self-protection awareness as well as educate and urge them to strictly follow safety operation process.
The company shall improve its job site safety supervision and coordination capabilities to stop workers’ unsafe behaviors.
“2021.7.9” Job site vehicle accident at Anfen Road
Summary of the accident：
At about 9:30 am, July 9, 2021, at the district construction material waste recycle center of Hongkou District at No.580 Anfen Road, a worker of Shanghai Xunyan Environmental Technology Co. Ltd. was driving a forklift to move a garbage dumpster. Unfortunately the vehicle knocked down a waste sorter and killed the latter.
Direct cause：
The waste sorter was hit and dragged by the forklift.
Indirect cause：
Shanghai Xunyan Environmental Technology Co. Ltd. had many work safety management issues, such as lacking internal safety process regulations, lacking a complete work safety hazard check and insufficient safety training among its employees.
Although the company had a “Safety Process Regulation for Forklifts”, it did not specify the speed limits on forklifts making turns; it did not standardize its forklift working environment requirement based on the real condition; it did not take into account various factors that might increase accident possibility, such as bad visibility due to its dusty air condition, lack of proper lighting, complicated driving routes and lack of proper sign boards;
It did not have a customized work safety hazard review system which fits the nature of its operations; Xunyan company did not undertake regular safety checks to review its safety education effectiveness, employee compliance of internal rules and regulations as well as safety level of its operational environment.
Its work safety education programs were superficial. Relevant managerial personnel were not clear about safety operation protocols for key positions; workers lacked proper understanding of the hazards of the job site and their positions.
Punishment for the accident：
The forklift driver was transferred to a judicial organ for investigation of criminal responsibility. Hongkou District emergency management bureau imposed administrative penalties upon Shanghai Xunyan Environmental Technology Co. Ltd. and its principal leader.
Recommendations：
Shanghai Xunyan Environmental Technology Co. Ltd. shall learn from this accident and take on its workplace safety responsibility. The company shall design a set of complete work safety rules and regulations, hire full-time or part-time professional work safety supervising personnel according to the requirement of laws and regulations, and design customized standard operational processes for its positions according to its operational reality. The company shall regularly undertake blanket work safety hazard checks, improve its working environment and properly arrange its workplace, as well as scientifically design its workloads and shifts. The company shall not ask its workers to overwork and must guarantee they can have enough rest.
The parent company of Shanghai Xunyan Environmental Technology Co. Ltd. shall also amplify the lessons learned from this accident to improve its workplace safety management, build a robust safety responsibility system and rate its employees’ performances against it. It shall regularly organize work safety training programs for managerial personnel of its subordinate companies. It shall also regularly supervise its subordinate companies’ work safety management level.
The owner of the district construction material waste recycle center must enhance its work safety management and ensure that the project contractor have a robust safety responsibility system and standard process regulations, identify and eliminate safety hazards, and take on their work safety responsibilities.
Hazard warnings
➤Recently more electric shock accidents and sudden deaths have happened. Businesses and organizations shall raise their employees’ safety awareness, better manage their electrical equipment and require employees to strictly follow safety protocols and regulations to prevent electric shocks. During extreme hot days, businesses and organizations shall also flexibly arrange workloads and shifts to protect workers from heat-strokes.
We are now entering the season of poisoning and suffocating accidents, which are likely to happen in confined space works. In recent years, there have been accidents with multiple injuries and deaths which were caused by poisoning and suffocating as well as unprofessional rescue attempts. We must learn from those lessons. In industries prone to such hazards, such as food processing, wastewater processing, urban construction and underground operations, businesses and organizations must enhance their safety management for confined space operations as well as undertake safety prevention measures such as ventilation, gas detection and monitoring. Once accident happens, they shall organize rescue work professionally.
